Flex Health Solutions is seeking dedicated and compassionate Physical Therapists to join our dynamic team at our Rehabilitation Center located in Meriden, Connecticut. This full-time position is vital in enabling our clients to restore their functional abilities and improve their quality of life through tailored rehabilitation programs and innovative treatment methodologies. This position is based onsite at our state-of-the-art Rehabilitation Center situated in Meriden, Connecticut. • Conduct thorough Physical Therapy evaluations for patients, particularly focusing on children, to assess their functional abilities and limitations.
• Develop and implement individualized treatment plans that address each patient’s specific needs and goals, utilizing evidence-based practices.
• Regularly monitor and reassess clinical progress, adapting treatment plans as necessary based on response to therapy and changing patient conditions.
• Maintain accurate and timely documentation, including SOAP notes, progress reports, and care plans in our electronic health record system to ensure compliance and continuity of care.
• Educate patients and their families about recovery processes, potential challenges, and self-management strategies to promote a collaborative approach to care.
• Meet departmental utilization goals and participate in organizational objectives aimed at enhancing operational efficiency and patient satisfaction.
• Engage in interdisciplinary collaboration with peers across various healthcare specialties to coordinate care for clients receiving multifaceted therapy services.
• Represent and advocate for the company culture while serving as an ambassador for our core values in every interaction.

This role focuses primarily on treating a diverse range of patients, including children with developmental challenges and those recovering from injuries. You'll have the opportunity to work with clients from various backgrounds, which requires cultural sensitivity and a commitment to equitable care. Your work will directly impact their rehabilitation journey, helping them achieve independence and improve their overall health outcomes.

• A Master’s degree or Doctor of Physical Therapy (DPT) from an accredited program is mandatory.
• Current and unrestricted state licensure as a Physical Therapist is required.
• Willingness to conduct evaluations and provide treatments both in person and via telehealth, aligned with clinical guidelines.
• A strong desire and commitment to servicing a diverse patient population, encompassing various ages, ethnicities, and abilities.
• Ability to adapt and thrive in a fast-paced, evolving environment.
• Demonstrated teamwork and collaboration skills, along with a focus on exceptional patient service.

• Experience in pediatrics or working with diverse populations is highly desirable.
• familiarity with electronic health record (EHR) systems and documentation practices.
• Previous experience in a rehabilitation center or similar healthcare facility.
