Location: Columbia Falls, Flathead County and surrounding areas 

 

Granite Peak Home Health is a leading home health agency providing high-quality, patient-centered care in Columbia Falls, Montana. We specialize in skilled home health services, helping patients recover safely at home while improving independence and quality of life.

 Position Summary

We are seeking a licensed Occupational Therapist (OT) for a home health position. This role focuses on adult and geriatric patient care, providing in-home therapy services to individuals recovering from illness, injury, or surgery.

This is a great opportunity for an OT looking for flexible scheduling, autonomy, and meaningful one-on-one patient care in a home health setting.

Key Responsibilities
  • Perform occupational therapy evaluations and OASIS assessments (if applicable)

  • Develop and implement individualized treatment plans

  • Provide skilled therapy services in patients’ homes

  • Improve patients’ ability to perform activities of daily living (ADLs) and instrumental ADLs (IADLs)

  • Recommend adaptive equipment, home modifications, and fall prevention strategies

  • Collaborate with an interdisciplinary healthcare team (RN, PT, SLP, caregivers)

  • Complete accurate EMR documentation in compliance with Medicare and home health regulations

  • Monitor patient progress and adjust care plans accordingly

Qualifications
  • Active Occupational Therapist license in Montana (MT OT License)

  • Degree from an accredited Occupational Therapy program

  • Minimum 1 year experience in home health, skilled nursing, rehab, or acute care (preferred)

  • Knowledge of Medicare guidelines, OASIS documentation, and home health compliance (preferred)

  • Strong skills in patient assessment, care planning, and clinical decision-making

  • Valid driver’s license, reliable transportation, and ability to travel between patient homes

  • Current CPR/BLS certification
