What is a First Choice Primary Care provider?

A First Choice Primary Care provider is a healthcare professional or clinic focused on delivering comprehensive, accessible, and ongoing primary care services to patients. These providers manage general health concerns, preventive care, chronic disease management, and coordinate specialist referrals if needed. They serve as the first point of contact for most health issues and help ensure patients receive the appropriate care throughout their healthcare journey.

To thrive as a Primary Care Provider, you need a medical degree (MD, DO, NP, or PA), clinical experience in family or internal medicine, and relevant licensure. Familiarity with electronic health records (EHRs), diagnostic tools, and chronic disease management systems is crucial. Strong interpersonal skills, cultural competence, and patient-centered communication distinguish top performers in this role. Job description

Primary Care Physician

Location: New Albany, IN

Salary Range: $205,000 - $300,000

Why Join Us?

We're on a mission to transform primary care, and we're looking for a compassionate and skilled Primary Care Physician to help lead the charge. In this role, you'll be at the heart of a collaborative, multidisciplinary team that delivers exceptional, patient-focused care. We prioritize quality over quantity, ensuring you have the time and resources to truly connect with your patients and improve their lives.

Our senior-focused, value-based care model is designed to make a meaningful impact by offering concierge-level services, coordinated care, and strong ties to the community. If you're ready to practice medicine in a way that allows you to make a real difference, we'd love to have you join us.

What You'll Do

  • Provide personalized, high-quality medical care to patients following best practices and evidence-based guidelines.
  • Regularly review patient data and medical records to monitor and improve health outcomes.
  • Work closely with your care team to coordinate referrals, transitions of care, and other patient needs such as durable medical equipment and home health services.
  • Contribute your expertise to daily care team huddles, fostering a culture of collaboration and excellence.
  • Partner with leadership to create a supportive and professional clinic environment that emphasizes teamwork.
  • Ensure timely and thorough documentation of patient care while leveraging quality-based coding to enhance care delivery.
  • Engage in ongoing quality discussions with the Regional Medical Director to refine processes and outcomes.
  • Play an active role in identifying and implementing opportunities to expand services and improve patient care.
  • Participate in the local on-call program as needed, ensuring seamless care for patients.
  • Dedicate your time fully to clinical care, including both patient-facing interactions and related administrative duties.

What You Bring Required:

  • Medical degree (MD or DO) from an accredited program.
  • Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA).
  • An active, unrestricted medical license in your state (or willingness to obtain one).
  • Strong interpersonal skills and a passion for building meaningful patient relationships.
  • A collaborative mindset, fully committed to team-based care.
  • Comfort with value-based care practices and electronic medical records (EMR).
  • Compliance with Tuberculosis (TB) screening requirements.

Preferred:

  • Active DEA license.
  • Medicare and Medicaid provider credentials.
  • Two or more years of experience in primary care or a value-based care environment.
  • Familiarity with Medicare guidelines, HEDIS metrics, and coordinated care best practices.

What We Offer

  • A guaranteed competitive base salary with additional quarterly performance bonuses.
  • Comprehensive benefits starting on day one, including health, dental, and vision insurance.
  • Generous CME allowance and dedicated time off to pursue professional development.
  • Relocation assistance and sign-on bonus options.
  • Malpractice insurance with occurrence-based coverage.
  • 401(k) retirement plan with employer matching.
  • Life and disability insurance.
  • Generous paid time off and holidays.
  • Minimal on-call responsibilities, allowing for better work-life balance.

Your Work Schedule

  • Full-time position, 40 hours per week.
  • Monday to Friday, 8:00 AM to 5:00 PM.

Why This Role Is Different

This isn't just another primary care role. You'll have fewer patients to see each day, giving you the opportunity to spend quality time with each individual and make a real impact. Supported by a robust care team and advanced tools, you'll practice medicine the way it should beโ€”with the time and resources to focus on delivering exceptional care.

If you're ready to elevate your practice and work in a collaborative, patient-first environment, we invite you to apply. Be part of a team that's changing livesโ€”one patient at a time.
