Job description

Job Summary

Provides expertise in caring for patients with life-limiting, serious illness and their families. Responsible for synthesizing complex data to assess, diagnose, implement, and coordinate plans of care that assist with symptom control, functional status, and enhanced communication. The overall goal is to improve the quality of life for every patient and family we have the privilege of serving.

What You Should Know About the APRN Role
  • This is a full-time remote overnight position with the hours of 8:00 PM – 8:00 AM.
  • Candidates must reside in Ohio and be located within a reasonable distance of one of our office locations in Dayton, Newark, or Wooster.
  • Evening, weekend, and holiday coverage may be required based on operational needs.
  • This role provides telephonic and virtual clinical support for hospice and palliative care patients during overnight hours.
  • We provide superior care and compassionate service to patients and families throughout their end-of-life journey. Only those with a heart for hospice will succeed.
Qualifications
  • Master of Science in Nursing (MSN).
  • Two years of clinical experience as an Advanced Practice Registered Nurse.
  • Active Registered Nurse license in the State of Ohio without board actions.
  • Board certification as an Advanced Practice Registered Nurse by a nationally recognized certifying agency.
  • Active, unrestricted DEA license.
  • Eligible to maintain adequate malpractice coverage.
  • Certification in Hospice & Palliative Medicine (ACHPN) preferred; required within two years of employment (eligible upon completion of 500 hours of hospice/palliative experience).
  • CPR Certified.
  • Computer skills sufficient to properly document services and patient care.
  • Reliable high-speed internet and a dedicated workspace suitable for remote work.
  • Valid Ohio driver's license with a safe driving record (if occasional travel is required).
  • State minimum automobile insurance coverage.
  • Must be able to pass a criminal background check.
  • Must be able to pass a 10-panel drug screen.
