We are seeking a compassionate, motivated Family Nurse Practitioner (FNP) to provide high-quality, patient-centered primary care to individuals and families across the lifespan. This role is ideal for a clinician who is adaptable, collaborative, and committed to delivering culturally sensitive services in a friendly, professional setting.
Key Responsibilities
  • Provide comprehensive primary care services across the lifespan, including assessment, diagnosis, treatment, and follow-up care
  • Prescribe medications and manage treatment plans within scope and full prescribing authority
  • Document care accurately and timely in the electronic health record (EHR)
  • Collaborate with physicians, nurses, behavioral health providers, and other clinical team members to support quality patient outcomes
  • Deliver exceptional patient experience by consistently maintaining a friendly, professional, and public-facing presence
  • Support clinic workflows and contribute ideas to improve care delivery, efficiency, and patient satisfaction
  • Maintain compliance with organizational policies and applicable state/federal requirements
Required Qualifications
  • Minimum 1 year of experience practicing as a Family Nurse Practitioner (relevant prior medical/clinical experience considered)
  • Completion of an accredited Nurse Practitioner program
  • Current, unrestricted Arizona APRN license as a Certified Nurse Practitioner (CNP) with a Family/Individual Across the Lifespan focus and full prescribing authority
  • Board Certified as a Family Nurse Practitioner (AANP or ANCC)
  • Initiative, creativity, problem-solving ability, adaptability, and flexibility
  • Ability to collaborate with professional colleagues to provide quality care
  • Consistently present a friendly, welcoming, and professional public-facing presence
  • Strong verbal, written, and communication skills
  • Knowledge of Windows environment and/or Electronic Health Records (EHR) systems
  • Ability to provide culturally sensitive services
  • Must have or be able to obtain a valid Fingerprint Clearance Card
  • Must possess reliable transportation, a valid driver's license, and proof of auto insurance
  • BLS certified or ability to be trained within 5 business days of hire date
  • Experience working in community health, integrated care, or with diverse/underserved populations preferred
  • Bilingual preferred
Pre-Employment / Onboarding Requirements
  • TB screening record and PPD/QuantiFERON/Chest X-ray upon hire (as applicable)
  • Vaccinations/titers/declination forms: Flu, Hep B, MMR, Tdap, Varicella
