Job description

Luther Park Community is seeking a skilled, detail oriented, talented and experienced RN for the position of ADON and Infection Preventionist. We are combining the two positions into one to create this unique opportunity for the right RN. This position is located at our Trinity Center which specializes in Long-Term Care, Skilled Care and Alzheimer's and Dementia care. The Trinity Center at Luther Park Community is a 120 bed facility with 60 CCDI beds with an overall care philosophy of serving those in the dementia disease process.
The ADON responsibilities will involve clinical management leadership and direction to nursing personnel through planning, organizing and carrying out nursing services and will orientate all licensed personnel to the facility regardless of shift. The Infection Preventionist responsibilities will be to lead and implement the infection prevention and control program to control the spread of infections and/or outbreaks by providing education to staff, residents and visitors as needed. This position will be responsible for instructing and supervising nursing personnel and comply with established policies and procedures.
RESPONSIBILITIES:
ADON
• Attend daily clinical meetings, resident care plan meetings and monthly QA meetings
• Supervise daily nursing operations
• Evaluate individual care plans through resident care conferences
• Apply conflict resolution techniques workingwith staff conflict or family inquiries and ensure follow-up has been conducted
• Keep the DON informed of resident status changesor other matters related to resident care or staff concerns
• Coordinate admissions/discharges and ensureall aspects of admissions and discharges have been completed
• Monitor that physician/family are informed ofresident status change
• Monitor physician orders
• Monitor monthly medication review and follow-upas needed
• Actively participate in RN/LPN and CNA/CMAmeetings
• Review fall incident reports ensuringappropriate follow-up and interventions are in place and care plans areupdated
• Attend Monthly weight meetings and notify thephysician, family of weight changes and ensure all interventions suggested by theDietitian are in place and documentation completed
• Maintain reports and data as requested by theDON
Infection Preventionist:
• Develop and implement ongoing infection prevention and control program
• Develop and implement written policies and procedures
• Complete infection control audits for new hires and other employees
• Complete weekly NHSN reporting
• Implement annual infection control risk assessment process
• Provide education related to infection prevention principles, policies and procedures to staff, residents and families (where appropriate)
• Participate in on-call rotation
Supervises:
RN's,LPN's, CMA's and CNA's
Requirements:
Graduate of an accredited schoolof nursing with a current state of Iowa RN licensure
Bachelor's degree in Nursing is preferred
Minimumof 1-3 years of RN experience, preferably in a long-term care setting
Two years of previousmanagement or supervisor experience preferred
Knowledgeableof MDS process
Knowledgeableof State and Federal regulations
Infection preventionist preferred
