Job Responsibilities

This position provides care to assigned residents in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized resident care plans.

Completes the assigned portions of the MDS for Nursing. Signs the MDS forms to signify completion of the process. Maintains residentsโ€™ self-esteem, privacy and confidentiality of personal information. Assists to maintain safe, pleasant, orderly resident living areas. Completes nursing portion of MDS and reviews for completion and validity of entire process. Queries other members for accuracy of MDS; and provides final quality check prior to MDS transmission. Receives and shares information. Observes and reports residentsโ€™ conditions. Communicates with physicians using center/campus systems. Directs and assists with co-worker, resident, family/significant other education. Provides support and recognition to co-workers related to completion of the RAI / MDS process. Uses the EMR accurately.

Qualifications

Graduate of an accredited school of nursing. RN nursing licensure for state of practice is required. Must be certified through AANAC as a Resident Assessment Coordinator (RAC-CT) within 6 month of hire.

Ability to communicate and comprehend.

Knowledge of the RAI/ MDS process; knowledge of basic nursing skills.
