Job description

Job Title

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until Sept 30, 2026. The initial cut-off date for referral of eligible applications will be August 24, 2026. Eligible applications received after that date will be referred at regular intervals on an as-needed basis until positions are filled. All referred applicants receive a final notification once a selection decision has been made.

Major Duties Include But Are Not Limited To:
  • Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
  • Provides cross-coverage for other CLC Nurse Managers and nursing needs as organizational needs arise.
  • Oversees and directs staff orientation, competency development, ongoing training, and performance evaluation for all nursing personnel in the CLC.
  • Function as a clinical role model, educator, supervisor, and leader who supports a culture of excellence in long-term, restorative, hospice, memory care, and short-stay rehabilitation services.
  • Collaborate closely with interdisciplinary team members to remove barriers, improve processes, and implement innovative, evidence-based clinical and operational practices that support resident-centered outcomes.
  • Equips staff with knowledge and tools to deliver culturally competent, trauma-informed, inclusive care, reinforcing a culture of ownership and responsibility for high-quality, cost-effective outcomes.
  • Ensures compliance with all accreditation and regulatory standards relevant to long-term care environments, including the Joint Commission, OIG, Ascellon, CMS requirements applicable to CLCs, and other agency-wide directives.
  • Functions as a clinical expert.
  • Responsible and accountable for all elements of the nursing process when providing or supervising direct patient care.
