Summary
Under general supervision, performs work of moderate difficulty in developing and providing public health education, conducting outreach, and connecting community members to services; performs related work as assigned. This is a grant funded position, and dependent upon the availability of grant funds.
Essential Job Functions

- Conducts outreach to individuals, families, schools, and community organizations to promote healthy behaviors and program participation.
- Delivers evidenced-based and culturally appropriate education sessions on topics such as adolescent health, substance use, naloxone, tobacco use prevention and cessation, chronic disease prevention, and emergency preparedness.
- Assists in planning, coordinating, and evaluating program activities and educational events.
- Provides support, and referrals to healthcare and social service providers as appropriate.
- Establishes and maintains cooperative working relationships with schools, healthcare providers, community partners, and other stakeholders.
- Maintains accurate records of outreach, education sessions, and client interactions; prepares required reports.
- Identifies barriers to care and advocates for improved access to health and wellness services.
- Participates in required trainings to maintain current knowledge of public health best practices and community resources.
- Performs other related duties as assigned.

Desirable Qualifications
High School diploma or G.E.D.; AND three years' experience working in a health field with one year of general office experience; AND a valid driver's license.
