Registered Nurse (RN), Home Health Evening Triage

As a Home Health Evening Registered Nurse (RN): is responsible for triaging all patient and patient family phone calls for the Agency between the evening hours Monday through Thursday, assessing the nature of the call and taking appropriate action. In addition, the RN is responsible for an assignment of patients for the evening hours, responsible for identifying patient/family needs utilizing the nursing process of ongoing assessment, planning, intervention, implementation and evaluation, and is responsible for identifying patient/family needs and providing quality care/services in accordance with the attending physician's orders, plan of care and Agency policies and procedures. Additionally, the candidate is responsible for performing patient care on an intermittent basis, in the patient's place of residence, in close coordination with other disciplines and caregivers involved in the patient's care.

***$20,000 SIGN-ON/RETENTION BONUS AVAILABLE FOR QUALIFIED CANDIDATES***

Shift: Monday through Thursday 1pm to 11pm

Qualifications:
  • Have a current PA RN License
  • Have a valid PA driver's license and reliable transportation
  • Have 2 years of acute care or long-term care experience
  • Have excellent time management skills
  • Have strong verbal and written communication skills
  • Have strong teaching skills
  • Have ability to work independently and within a multidisciplinary team
  • Have ability to pass PA Child Abuse History Clearance
  • Have ability to pass Criminal Background Check
  • Have ability to meet physical requirements of position
