Concierge Home Care is seeking an experienced Registered Nurse (RN) to provide skilled evening home health visits throughout Clay County. This role is ideal for nurses who enjoy working independently while helping patients remain safely at home.

Schedule

Monday-Friday

4:00 PM-10:00 PM

No overnight shifts.

Coverage includes:

• Evening Start of Care admissions
• Skilled nursing visits
• Urgent nursing visits (Up to 10:00 PM)
• Resumption of Care visits

Minimum Productivity:
24 points per week

What You'll Do

Provide skilled nursing care to patients in their homes while ensuring continuity of care after normal business hours.

Responsibilities include:

• Complete Start of Care (SOC) admissions
• Skilled nursing revisits
• Resumption of Care (ROC) visits
• PRN and urgent nursing visits
• Perform comprehensive patient assessments
• Medication reconciliation
• Develop and update individualized care plans
• Wound care
• Disease management education
• Catheter care
• IV starts
• Foley catheter management
• Nephrostomy care
• Infection prevention education
• Monitor urinary retention
• Post-surgical urology care
• Coordinate care with physicians and interdisciplinary teams
• Complete OASIS documentation and EMR charting
• Participate in rotating evening/weekend on-call coverage

What We're Looking For

Required:

• Active Florida RN License or Compact RN License
• Valid driver's license
• Reliable transportation
• Current auto insurance
