Procedures- standard urology procedures in the clinic and operating room

Required clinical activity in the last 24 months (case logs)- Applicant must provide documentation (case list) of provision of surgical services (100 procedures) representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date, and facility where performed.
