e4health is seeking an experienced Epic Healthy Planet / Value-Based Analytics Consultant to support healthcare organizations in optimizing population health, value-based care, quality performance, and clinical outcomes through the effective use of Epic Healthy Planet and related analytics capabilities.


The ideal candidate will have hands-on experience with Epic Healthy Planet, population health management, value-based care programs, quality measures, risk stratification, care management, and healthcare analytics. This individual will work closely with clinical, operational, quality, finance, and IT stakeholders to translate value-based care requirements into actionable analytics, workflows, dashboards, and reporting solutions.


Key Responsibilities

• Provide functional and analytical expertise for Epic Healthy Planet implementations, optimization, and ongoing support.

• Analyze and translate value-based care requirements into Epic Healthy Planet workflows, reporting, and analytics solutions.

• Support population health initiatives focused on quality, cost, utilization, outcomes, and patient engagement.

• Develop and optimize analytics supporting value-based contracts, accountable care, and population health programs.

• Work with clinical and operational stakeholders to define key performance indicators (KPIs) and actionable population health metrics.

• Support reporting and analytics for CMS, HEDIS, MIPS, ACO, Medicare Advantage, Medicaid, and other value-based care programs.

• Leverage Epic Healthy Planet data to identify care gaps, high-risk populations, utilization trends, and opportunities for clinical intervention.

• Support risk stratification and segmentation of patient populations based on clinical, financial, utilization, and quality measures.

• Develop and analyze care gap, registry, quality measure, and population health reports.

• Partner with clinical teams, care managers, quality teams, and physician leadership to translate analytics into actionable interventions.

• Analyze performance against value-based contract benchmarks and identify opportunities to improve quality and financial performance.

• Support development of dashboards and executive-level reporting related to population health and value-based care.

• Assist with data validation, reconciliation, testing, and troubleshooting of Healthy Planet analytics and reporting.

• Collaborate with Epic application teams, reporting/analytics teams, data analysts, and organizational leadership.

• Participate in requirements gathering, workflow analysis, solution design, testing, training, and go-live support.

• Evaluate existing population health processes and recommend opportunities for workflow, analytics, and operational improvement.

• Support data-driven strategies designed to improve patient outcomes while reducing avoidable utilization and cost.

Value-Based Analytics Experience

The successful candidate should demonstrate experience using healthcare data and analytics to support value-based care performance. Experience should include several of the following:

• Quality measure performance and gap analysis

• HEDIS and CMS quality measures

• ACO and Medicare Shared Savings Program analytics

• MIPS/MACRA reporting and performance

• Risk adjustment and patient risk stratification

Requirements:

• 5+ years of healthcare IT, clinical informatics, population health, or healthcare analytics experience.

• At least 5 years of demonstrated experience with Epic Healthy Planet project work.

• Experience supporting Epic Healthy Planet implementations or major optimization initiatives.

• Strong understanding of value-based care and population health analytics.

• Experience translating clinical and operational requirements into analytical and reporting solutions.

• Knowledge of healthcare quality programs and performance measures.

• Strong analytical, problem-solving, and data interpretation skills.

• Ability to communicate complex healthcare analytics concepts to both technical and non-technical stakeholders.

• Experience working with physicians, clinical leadership, quality teams, care management, finance, and IT.

• Strong project management and documentation skills.

• Epic certification in Healthy Planet required.

Preferred Experience

• Experience with Clarity and/or Caboodle.

• SQL and healthcare data analytics experience.

• Experience with Tableau, Power BI, or other enterprise analytics platforms.

• Experience with CMS, HEDIS, ACO, MIPS, Medicare Advantage, or Medicaid value-based programs.

• Experience with risk adjustment, RAF/HCC methodologies, or actuarial analytics.

• Experience with payer-provider value-based contracts.

• Experience developing executive dashboards and performance scorecards.

• Experience working within large health systems or integrated delivery networks.

PHYSICAL DEMANDS OF THE ESSENTIAL FUNCTIONS:

Sitting, talking, hearing and near vision are required over 90% of the time. Feeling is required over 90% of the time and reaching is required about 50% of the time. The ability to travel to field sites may be required up to 15% of the time.

WORKING CONDITIONS WHILE PERFORMING ESSENTIAL FUNCTIONS:

Over 90% of the time is spent indoors, with protection from weather conditions. Exposure to noise levels that may be distracting or uncomfortable is present in only unusual situations.


PAY RANGE

Pay range for this position is $75-85/hr
