Job description

Job Description:

The EPC Psych Case Manager position will receive referrals from both inpatient and outpatient services. Clients can be either EPC or voluntary referrals. This person will be responsible to perform an Intake Assessment and Individualized Program Plan for each client. Provide on-going support, advocacy, and crisis management follow-up services for referred patients. Length of service may be up to six months in duration.

EDUCATION: Bachelors Degree in a Mental Health related field (Social Work, Psychology, etc.) required.

EXPERIENCE: Preferred 2 years of clinical experience in case management related activities for persons with severe and persistent mental illness. Preferred clinical experience with substance abuse treatment issues.

CERTIFICATIONS/LICENSES: Valid NE Drivers License required. Basic Life Support (BLS) through American Heart Association (AHA) required within 3 months of hire. CPI required within 3 months of hire.
