Clinical Assessment Manager (UAS Experience Required)
Bronx, NY

We are seeking an experienced Clinical Assessment Manager to support in-home participant assessments and ensure the effective and efficient completion of all tasks associated with the clinical assessment process.

This position plays a key role in participant onboarding, reassessments, and continuity of care within a managed care/community-based setting.

Position Details
  • Full-Time
  • Monday–Friday
  • 8:30 AM–5:30 PM
  • 1-hour lunch
  • Orientation in the Bronx
  • Temp opportunity with potential for permanent hire based on performance and business needs
Responsibilities
  • Conduct in-home participant assessments throughout Brooklyn
  • Complete UAS-NY assessments accurately and efficiently
  • Perform comprehensive clinical assessments and medication reviews
  • Conduct post-hospital discharge evaluations
  • Support clinical onboarding and reassessment processes
  • Collaborate with interdisciplinary teams to coordinate participant care
  • Maintain timely and accurate documentation
Work Environment
  • Field-based role
  • Conducting in-home participant assessments throughout the designated service area
Qualifications

Required

  • Prior UAS experience
  • Active New York RN license
  • Strong assessment and clinical documentation skills
  • Experience conducting community-based or homecare assessments
  • Ability to work independently in the field

Preferred

  • Managed care, MLTC, or homecare experience
  • Experience with post-discharge and transitional care assessments
