Job description

We Are Inspired to Serve. Join us!Part of a nationwide family of Continuing Care Retirement Communities is seeking a Social Services Assistant. This position is
responsible for directing the social service functions which include the planning, development and implementation of appropriate programs designed to address the emotional and psychological needs of campus residents in multiple levels of care and their families. This position is responsible for actively participating in the resident assessment process and in assisting the facilitating of inter-facility transfers. This position serves as a resident advocate within the campus.

ESSENTIAL RESPONSIBILITIES:

  • Assist as the facility representative with prospective residents, families and discharge planners for persons in need of skilled nursing care.
  • Assist with orientation of residents to the skilled nursing facility and services.
  • Collect and accurately record admission data including social, medical and financial information and address questions and concerns and family adjustment.
  • Maintain current documentation in resident's health record as it relates to the social services care component of the comprehensive care plan for each resident as identified by the interdisciplinary team.
  • Participate in resident care planning meetings for ongoing departmental communication and problem-solving of delivery of Social Services to residents and families.
  • Maintain contact with hospital discharge planners to assure continuity of care and continued physician service upon transfer.
  • Assist residents and/or families in selecting new physicians as necessary.
  • Assist in the communication of appropriate information to departments regarding admission or discharge.
  • Assure that appropriate care plan documentation as required by regulations and need is complete.

EDUCATION AND WORK EXPERIENCE:

  • Required Degree: High School Diploma or the equivalent
  • Preferred Degree: Associates Degree
  • Certificate(s): Certifications in related areas are highly desirable.
  • Experience: Entry Level

KNOWLEDGE, SKILLS, AND ABILITY:

  • Effective communication, organizational skills, and interpersonal skills
  • Strong documentation skills, keep files updated, establish progress reports and collaborate with others in your filed to organize a comprehensive treatment or management plan for clients/patients. Understand and follow all regulatory and corporate policies related to confidentiality of the resident's medical, financial and personal information.

PHYSICAL REQUIREMENTS:

  • Sufficient physical ability and mobility to work in an office/community setting.
  • Ability to stand or sit for prolonged periods of time.
  • Ability to occasionally stoop, bend, kneel, crouch, reach, and twist.
  • Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 50 lbs.
  • Ability to operate various equipment base on department and weather conditions
  • Engaging in repetitive movements of wrists, hands, and fingers - typing and/or writing.
  • Clarity of Vision:
    • For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers.
    • For far visual acuity, Clarity of vision at 20 feet or more. This is not just the ability to see a person or object, but the ability to recognize features as well.
    • For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.

Compensation Pay Range:

$17.52 - $23.06 per hour

Reasonable Pay Estimate

A reasonable estimate of the pay range for this position is $17.52 - $23.06 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
