Nurse Practitioner/Physician Assistant Primary Care Alf/Snf

The Nurse Practitioner/Physician Assistant Primary Care Alf/Snf will provide primary care services to patients in an assisted living facility and skilled nursing facility. The successful candidate will work collaboratively with physicians, nurses, and other healthcare professionals to provide high-quality care to patients.

Responsibilities:

  • Conduct comprehensive assessments of patients' health status
  • Develop and implement treatment plans
  • Order and interpret diagnostic tests
  • Prescribe medications and other treatments
  • Provide patient education and counseling
  • Collaborate with physicians and other healthcare professionals to provide coordinated care
  • Maintain accurate and complete medical records
  • Participate in quality improvement initiatives
  • Adhere to all applicable laws, regulations, and standards of practice

Qualifications:

  • Current licensure as a Nurse Practitioner or Physician Assistant in the state of Kentucky
  • Master's degree in Nursing or Physician Assistant Studies
  • Certification as a Nurse Practitioner or Physician Assistant
  • Experience in primary care, preferably in a long-term care setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Ability to maintain confidentiality and adhere to ethical standards
