• Collaborate with primary care providers to recognize and manage mental health and psychosocial issues, ensuring that clients receive holistic care.
• Conduct thorough patient intakes focused on diagnostic and functional evaluations, providing actionable insights and treatment recommendations to primary care providers.
• Deliver targeted, brief interventions for patients requiring mental health support, tailoring approaches based on individual needs and treatment goals.
• Determine the suitability of patients for primary care settings versus specialist referrals, advising appropriately on care pathways.
• Monitor patient progress through regular follow-ups and adjustments to treatment plans as necessary, ensuring ongoing support and efficacy of interventions.
• Develop and implement relapse prevention plans to assist patients in maintaining stability and avoiding setbacks.
• Identify patients at risk of deterioration in their mental health and implement preventive strategies in collaboration with the care team.
• Coordinate and monitor the delivery of health services in relation to behavioral health, linking patients with external providers when appropriate.
• Document all patient interactions accurately within treatment charts, keeping all relevant stakeholders informed about treatment progress and adjustments.
• Empower patients with self-management skills by providing them with educational resources that enhance their participation in their treatment journey.
• Advise clinic management on potential areas for improvement within behavioral services and assist in program evaluations.
• Support patients in adhering to medical treatments prescribed by primary care providers, including strategies to manage medication side effects.
• Uphold the Code of Ethical Conduct for Clinical Professionals in all interactions and tasks.
• Perform additional duties as assigned, contributing to the overall success of the clinic.

You will work closely with varied patient demographics, each presenting unique mental health challenges and needs. The patients include individuals from different socio-economic backgrounds, many of whom face multiple barriers to health care. By addressing their psychological well-being, you will promote healthier lifestyles and improve their overall quality of life.

• A minimum of a Master’s Degree in Clinical Counseling is required.
• A current and valid LPC (Licensed Professional Counselor) License in Oregon.
• Strong foundation in mental health assessment, treatment planning, and various therapeutic methodologies, including family therapy, group therapy, and child psychotherapy.
• Excellent knowledge of DSM-IV diagnoses and behavioral intervention strategies.
• Bilingual proficiency in English and Spanish is preferred but not mandatory.
• Demonstrated mental and physical resilience to meet the demands of the position.
• Extensive understanding of how mental and physical health issues impact individuals, families, and communities.
• Familiarity with community resources and how they can be leveraged for patient support.
• Comprehensive understanding of psychodiagnostic assessments and therapeutic techniques.

• Experience working in a community health or multidisciplinary team setting is highly valued.
• Prior experience with electronic health records and related documentation systems is advantageous.

This position offers a competitive salary range of USD 87,000–105,000 per year, commensurate with experience and qualifications. Additionally, our center provides a comprehensive benefits package, including health insurance, paid time off, and potential retirement benefits.
