Key Responsibilities
  • Collaborate with primary care providers to recognize and manage mental health and psychosocial issues, ensuring that clients receive holistic care.
  • Conduct thorough patient intakes focused on diagnostic and functional evaluations, providing actionable insights and treatment recommendations to primary care providers.
  • Deliver targeted, brief interventions for patients requiring mental health support, tailoring approaches based on individual needs and treatment goals.
  • Determine the suitability of patients for primary care settings versus specialist referrals, advising appropriately on care pathways.
  • Monitor patient progress through regular follow-ups and adjustments to treatment plans as necessary, ensuring ongoing support and efficacy of interventions.
  • Develop and implement relapse prevention plans to assist patients in maintaining stability and avoiding setbacks.
  • Identify patients at risk of deterioration in their mental health and implement preventive strategies in collaboration with the care team.
  • Coordinate and monitor the delivery of health services in relation to behavioral health, linking patients with external providers when appropriate.
  • Document all patient interactions accurately within treatment charts, keeping all relevant stakeholders informed about treatment progress and adjustments.
  • Empower patients with self-management skills by providing them with educational resources that enhance their participation in their treatment journey.
  • Advise clinic management on potential areas for improvement within behavioral services and assist in program evaluations.
  • Support patients in adhering to medical treatments prescribed by primary care providers, including strategies to manage medication side effects.
  • Uphold the Code of Ethical Conduct for Clinical Professionals in all interactions and tasks.
  • Perform additional duties as assigned, contributing to the overall success of the clinic.

Qualifications
  • A minimum of a Masterโ€™s Degree in Clinical Counseling is required.
  • A current and valid LPC (Licensed Professional Counselor) License in Oregon.

  • Excellent knowledge of DSM-IV diagnoses and behavioral intervention strategies.

Compensation and Benefits
