As a Licensed Professional Counselor, you will have a diverse array of responsibilities, including:

• Collaborating with primary care providers to identify, treat, and manage mental health and psychosocial issues, ensuring an integrated approach to patient care.
• Conducting thorough patient intakes, executing diagnostic evaluations, and developing tailored treatment plans in accordance with established guidelines.
• Providing brief and focused interventions for patients who require mental health services, utilizing evidence-based therapeutic techniques.
• Advising medical staff on the suitability of continuing treatment within primary care versus referrals to specialty mental health services.
• Establishing follow-up protocols to monitor patient progress and adjusting treatment strategies as necessary.
• Creating relapse prevention plans to empower patients in maintaining stable functioning and preventing deterioration of their conditions.
• Identifying at-risk patients and collaborating with healthcare teams to create preventative strategies.
• Documenting patient progress and diagnostic information thoroughly and accurately within treatment charts.
• Providing valuable insights to the primary care team regarding patient needs, treatment updates, and behavioral strategies to enhance overall healthcare outcomes.
• Promoting self-management skills among patients, assisting them in taking an active role in their treatment and recovery process.
• Consulting with clinic management and interdisciplinary teams regarding best practices in behavioral health service delivery.
• Supporting patients in navigating and adhering to medical treatments prescribed by their primary care providers.
• Adhering to the Code of Ethical Conduct for Clinical Professionals while performing assigned duties.

Your clinical work will predominantly support a diverse patient population in the Denver area, including individuals facing a variety of mental health needs compounded by social determinants of health. As a vital member of our team, you will utilize your skills to connect with patients from different backgrounds, promoting holistic healing and well-being.

• A minimum of a Master’s Degree in Clinical Counseling from an accredited institution.
• Current LPC (Licensed Professional Counselor) License in the state of Colorado.
• Strong understanding of mental health assessment, treatment planning, and psychotherapeutic techniques.
• Thorough knowledge of developmental psychopathology and effective therapeutic systems, including family therapy and child psychotherapy.
• Ability to assess individual patient needs and adjust care plans to optimize outcomes.
• Proficient in maintaining clinical records and completing necessary reports with attention to detail.
• Ability to establish effective working relationships across diverse patient populations, families, and multidisciplinary teams.

• Bi-lingual proficiency in English and Spanish is preferred, enhancing communication with our diverse client base.
• Familiarity with community resources and the ability to leverage them for comprehensive patient care.
• Experience working in an integrated healthcare setting with a focus on collaborative care models.
• Exposure to mental health treatment channels pertaining to both acute and chronic conditions.

The salary for this position ranges between USD 87,000 and USD 105,000 annually, commensurate with experience and qualifications.
