• Conduct comprehensive bio-psychosocial evaluations to better understand the needs and backgrounds of patients, effectively identifying barriers to treatment and enhancing overall healthcare utilization.
• Collaborate with our multidisciplinary team to develop and implement individualized treatment plans utilizing evidence-based modalities, engaging in both individual and group therapy sessions.
• Document all social work activities accurately in patients’ medical records, adhering to departmental standards regarding frequency and content.
• Provide ongoing education and advocacy for patients and their families, empowering them to understand and navigate their treatments, while promoting self-determination within the community.
• Stay informed on community resources and services that can aid the client system, ensuring the highest level of support and outreach.
• Participate in professional development opportunities to stay current with industry trends, continually enhancing clinical skills and knowledge.
• Uphold the professional standards of practice according to the NASW Code of Ethics, ensuring a commitment to ethical and empathetic care delivery.

The Licensed Clinical Social Worker will focus on clientele that may include patients experiencing chronic illness, mental health disorders, or those undergoing end-of-life care. This requires sensitivity and an understanding of the complex emotional and psychosocial dynamics involved in these situations. Your role will be essential in addressing the unique challenges faced by these individuals and their families, offering empathetic support and facilitating access to necessary resources and services.

• Master’s Degree in Social Work from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) per the standards of the State Board of Social Work Licensure.
• Demonstrated understanding of the aging processes, human development stages, and cultural factors pertinent to patient care.
• A year of clinical experience in a healthcare or mental health setting is preferred, with a strong emphasis on serving diverse populations.

• Familiarity with electronic health record (EHR) systems and remote telehealth platforms to efficiently manage patient information and care plans.

This position offers a competitive annual salary ranging from USD 85,000 to 100,000, depending on experience and qualifications.
