Our client is looking for RN's who want tojoin a growing team immediately.
Experience/Job Skills
  • Minimum 1 years clinicalpractice experience required, preferably in an acute care setting.

Essential Functions of Job
  • Assessment: Evaluates andidentifies an individual's needs an intensity of case management services.
  • Assessment: Performs datacollection and analysis of the patient history to include, but not limitedto, current medical status, demographics, nutritional assessment,medication assessment, financial assessment, environmental assessment,psychosocial assessment, cultural and religious assessment,
  • Planning: Interfaces with theindividual, individual's family, primary care physicians, and specialiststo develop the case management plan,
  • Planning: Acts as a patientadvocate and clinical liaison by identifying social needs and researchingcommunity resources available to meet patient's needs.
  • Planning: Appropriatelyactivates members of the care team based on member assessment,
  • Planning: Developsindividualized care plans focused on individual engagement, education, andself-management,
  • Planning: Able to alignmember's identified assessment problems with individualized andprioritized interventions and SMART goals,
  • Implementation: Implements apatient-centered interdisciplinary approach to carryout care plan,
  • Implementation: Act asfacilitator and coordinates of care, services, resources, and healtheducation specified in the planned interventions.
  • Evaluation: Monitors andevaluates the effectiveness of the care plan and updates as necessary,
  • Evaluation: Conducts ongoingfollow-up with the individual, family and/or family caregiver in theevaluation of the individual's status, goals, and outcomes,
  • Closure: Able to determineprogression towards goals and appropriately move patients through thecontinuum of care

Preferred qualifications
  • Experience with Interqualand/or Milliman criteria helpful but not required..
  • 1-year case managementexperience preferred.
