Job description

Professional Case-Management Services of America (PCSA) currently has a full-time entry level opening in the Conyers, GA and Tucket, GA areas and the surrounding counties.

We are seeking a dedicated and compassionate Social Service Case Manager specializing in working with the Intellectual and Developmental Disabilities (IDD) population, to join our dynamic team. This vital role involves empowering individuals with developmental disabilities by coordinating services, fostering community integration, and promoting independence. The ideal candidate will possess a strong background in social work, program development, and relationship management, demonstrating a commitment to enhancing the quality of life for those served. This paid position offers an opportunity to make a meaningful impact through comprehensive support and collaborative program management.

Responsibilities

  • Maintaining a caseload of 35 – 40 individuals of the Intellectual and Developmental Disability (I/DD) population.
  • Maintain a current knowledge of all DBHDD state standards, guidelines, policies and procedures as related to service provision to the individuals with disabilities
  • Communicate the mission and vision of PCSA to participants, families and Residential/Day Program providers along with other stakeholders when providing treatment planning for the individuals on your assigned caseload.
  • Attend meetings with families, providers or to the region when requested by the Area or General Manager
  • Availability to work 9:00-6:00 and 10:00-7:00

Skills

  • Computer experience with Word/Excel/PowerPoint and Outlook. "Required"
  • Expertise in program development tailored to individuals with developmental disabilities or behavioral health needs.
  • Proven ability to build lasting relationships with clients, families, service providers, and community stakeholders.
  • Time management skills to prioritize tasks and meet deadlines.
  • Be able to independently manage time to fill a productive workday while meeting established benchmarks.
  • Experience in social work or related fields with a focus on person-centered planning and advocacy.
  • To be detail-oriented, a requirement necessary for weekly documentation.
  • Must have access to a laptop and internet connection.
  • Ability to maintain confidentiality of information
