Job description

    Coordinates Coumadin and Lovenox anticoagulation for new and established clinic patients
    Gathers information from provider to obtain anticoagulation needs, diagnosis, desirable range and length of treatment.
    Monitors and coordinates lab testing and dosage instructions with the patient
    Obtains and enters lab orders in accordance with protocol
    Adjusts anticoagulation dose in accordance with the protocol or as instructed by provider 
    Provides teaching and reviews safety issues with patients and care givers
    Documents in CoagClinic reason for anticoagulation, diagnosis, desired range, laboratory data, dosage instructions, precautions, and follow up laboratory testing
    Initiates home INR monitor  testing with Alere, as instructed by provider
    Processes prescription refills in accordance with refill policy or as instructed by provider 
    Notifies provider of any adverse reactions or when excessive anticoagulation occurs
    Coordinates with other providers and patients when anticoagulation needs to be temporarily suspended for procedure
    Works with VNA, other Home Health Groups and independent labs
    Sets up new  home draws 
    Coordinates bridge therapy with Lovenox as directed
    Tracks delinquent anticoagulation patients and coordinates their follow-up requirements
    Performs other work related duties as assigned or requested.
 

    Graduate of an accredited school of nursing
    Currently licensed as an RN (P021) or LPN (P016) in the Commonwealth of Massachusetts
    Basic Life Support Certification for Health Care Providers required
    Strong interpersonal, customer service, time management skills required
    Experience with current office technology, proficient with computer office applications
    Prior work experience and excellent knowledge in anticoagulation preferred
    Ability to multitask in a busy clinic setting including excellent communication and phone skills
