Job Summary

Reporting to the Manager and Interprofessional Practice Chief of Occupational Therapy, Physiotherapy, the Advance Practice Leader – OT/PT will be responsible for clinical care, research, education, leadership, consultation and collaboration utilizing a dynamic approach that is flexible and targeted to the patient, family and organizational needs. The Advance Practice Leader – OT/PT will work collaboratively with interdisciplinary and leadership teams to develop, execute, facilitate and sustain scholarly interprofessional practice and exemplary patient care. The Advance Practice Leader – OT/PT will also work with the clinical managers and other interprofessional practice leaders to facilitate and maintain accountability for standardized clinical best practice and will be involved in shaping the strategic direction of the various clinical programs, as well as advocate for scholarly gerontological care with key internal and external stakeholders within the health care system.

Key Responsibilities

• Leads program development and new models of care delivery for the population served, in collaboration with the program leadership team. 
• Assumes a leadership role within the program and the interdisciplinary team to develop solutions and service delivery models that provide exceptional patient/family care.
• Develops, implements and evaluates, in a collaborative manner, a sustainable approach to ensure standardized, evidence-based interprofessional practice across the organization.
• Authors clinical policies and procedures and medical directives that facilitate evidence-based practice.
• Monitors Local Health Integration Network (‘LHIN’), Ministry of Health & Long-Term Care standards and relevant legislation/regulations for compliance and collaborates on strategies to address shortfalls.
• Takes a lead role in the implementation of Required Organizational Practices (‘ROP’) mandated by Accreditation Canada.
• Works collaboratively with other programs and services to ensure quality patient outcomes and effective transitions of patients between programs and care settings.
• Facilitates discipline-specific and interprofessional education across the continuum by developing a continuing education strategy for staff to enhance the academic environment through student placement opportunities in partnership with the Education Department.
• Contributes to the improvement of clinical outcomes based on best available evidence and efficient utilization of resources.
• Develops and delivers orientation materials and educational resources to support the onboarding and ongoing development of staff.
• Participates in relevant quality improvement and practice development working groups and supports change management initiatives to promote current best practices and advance organizational goals.
• Provides consultation for complex admissions and discharge planning, supporting patient flow, care pathways, and transitions across the continuum of care.
• Provides consultation and guidance on mobility-related equipment recommendations and liaises with hospital vendors to support patient needs, clinical requirements, and organizational priorities.
• Serves as a clinical and practice resource for frontline clinicians and leadership, providing guidance and support on complex practice issues, clinical decision-making, and service delivery
• Supports OT/PT externs as a key clinical resource to promote successful onboarding, role integration, learning, and program outcomes

Qualifications

• Master’s degree (post entry-level) in a Health Discipline is required or relevant clinical and academic experience
• Five (5) years recent related experience and expert practice skills
• Demonstrated experience in leadership and clinical field required
• Current Certificate of competence in good standing from the relevant Regulated Health Profession College (i.e., College of Occupational Therapists of Ontario (COTO), College of Physiotherapists of Ontario (CPO)) required
• Membership in the discipline-specific national and/or provincial association is an asset.
• Experience in adult education/curriculum development and delivery
• Ability to communicate effectively to direct and motivate team and others, maintaining a persuasive and credible presentation style at all levels of the organization.
• Effective stakeholder engagement, interpersonal skills and communication skills to ensure effective communication at all levels of the organization including staff, clients/residents and their families and other external contacts.
• Ability to distill complex information into clear, concise messages.
• Demonstrated knowledge of current practice particularly, with a geriatric population.
• Ability to work with inter-professional teams and patient- centered program management structure.
• Demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams

Compensation

Our salary ranges are structured to support progression, from learning the role to demonstrating full proficiency.

The salary range for this position is $102,072 to $127,590 per year.

Your placement within the target hiring range will depend on your job-related knowledge, skills, abilities, and relevant education and experience, as well as considerations such as market conditions and internal equity. The starting salary will be determined using these factors to ensure fairness and consistency across the organization.
