LPN- Licensed Practical Nurse Care Coordinator
Description:
The LPN Care Coordinator is accountable for assisting the Director of Nursing/Assistant Director of Nursing and Nurse Manager, RN in the management and leadership of a specific nursing unit. This position, under the direction of the Director of Nursing and RN Nurse Manager, assists in the evaluation of available resources to assure quality outcomes of care. They also assist in the direction and implementation of the nursing process. This includes assisting the Director of Nursing and RN Nurse Manager with systematic data collection, assisting with the development of care plan, and implementation of care plan and revision of plan. The Care Coordinator, LPN coordinates the management of the care of residents through a well-defined care delivery system
Qualifications:
  • Must have a current LPN registration in the state of Minnesota.
  • Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
  • Must possess the ability to communicate verbally and in writing when coordinating the care of the residents.
