Job description

MedCare Pediatric is seeking a Bilingual Patient Care Coordinator to support our Pediatric Home Health Therapy team. This role serves as the primary point of contact for families, therapists, physicians, and referral partners after therapy services are established, helping ensure exceptional customer service, timely communication, and efficient coordination of care.
The ideal candidate is highly organized, detail-oriented, and passionate about providing outstanding service while managing documentation, authorizations, and daily operational workflows in a fast-paced healthcare environment.
Key Responsibilities:

  • Serve as the primary contact for bilingual (English/Spanish) families regarding established therapy services.
  • Coordinate communication between families, therapists, physicians, referral sources, and internal departments.
  • Monitor therapy documentation to ensure timely completion and compliance with agency standards.
  • Assist with obtaining, tracking, and managing insurance authorizations and payer requirements.
  • Coordinate physician orders, plan of care documentation, and other required clinical paperwork.
  • Maintain accurate patient records within the electronic medical record (EMR) system.
  • Support quality improvement initiatives, documentation audits, and process improvements.
  • Identify workflow challenges and collaborate with leadership to improve efficiency and patient access.
  • Provide exceptional customer service while ensuring patients receive timely, coordinated therapy services.
