As a Licensed Clinical Social Worker at Relationship Renewal Therapy, you will:
- Conduct comprehensive assessments of the psychosocial needs of your senior clients, focusing on emotional, mental, and social factors that affect their well-being.
- Collaborate closely with clients and their families to create individualized care plans tailored to their specific goals, preferences, and challenges.
- Provide counseling and emotional support, helping seniors navigate life transitions such as retirement, loss, and health changes.
- Advocate for clients' rights and help them access community resources, ensuring they receive appropriate services and support.
- Work alongside a multidisciplinary team to coordinate care, engage in regular virtual meetings to discuss client progress, and synergize efforts to enhance outcomes.
- Monitor clients’ well-being through regular follow-up calls and virtual visits, assessing living conditions and identifying any concerns or risks.
- Facilitate communication among family members involved in client care, helping to resolve conflicts and ensuring a unified approach to support.
- Educate clients and their families about available community resources, fostering social engagement and improving their overall quality of life.
- Maintain detailed and accurate documentation of client assessments, care plans, and progress notes in compliance with ethical and regulatory standards.

By providing essential support services, you will help seniors navigate their unique life circumstances while enhancing their emotional and mental well-being. This population often faces multiple transitions, including health issues, loss of loved ones, and adjustment to new life stages, making your role critical to their lives.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work role, particularly in community health settings or geriatric care.
• Strong understanding of the principles of social work, especially concerning seniors and aging issues.
• Excellent interpersonal skills with the ability to empathize and connect with clients and families effectively.
• Proficient in the use of electronic health record (EHR) systems and familiar with digital communication methods.

While a minimum of two years of experience is required, applicants with additional experience in geriatric social work or community health clinics are preferred. Familiarity with crisis intervention techniques and an understanding of local resources for seniors will be advantageous.

We offer a competitive salary range between USD 60,000 and USD 100,000 annually, commensurate with experience and qualifications.
