Bright Star Counseling is seeking a dedicated and compassionate Senior Licensed Master Social Worker (LMSW) to join our team in providing exceptional support for seniors. This full-time, remote position focuses on collaborating with seniors and their families to enhance their quality of life through individual care plans, advocacy, and coordination of services.

This position primarily operates in a remote setting, allowing flexibility in work-life balance while maintaining strong connections with our clients. Despite the virtual nature of the role, the social worker will engage regularly with clients through video calls, phone consultations, and remote assessments. This setting requires adaptability to ensure that clients receive continuous care, with occasional in-person visits or community outreach activities as necessary.

• Conduct in-depth psychosocial assessments of seniors to identify needs related to mental, emotional, and social health.
• Develop and implement individualized care plans in collaboration with clients and their families, addressing unique goals and preferences.
• Provide counseling services to seniors and their families, supporting them through challenging transitions such as illness, loss, or changes in living conditions.
• Advocate for clients by connecting them with community resources, ensuring access to vital services including healthcare, transportation, and social engagement programs.
• Work closely with an interdisciplinary team including nurses, caregivers, and healthcare professionals to enhance comprehensive care delivery.
• Conduct follow-up visits to assess changes in clients’ living conditions, monitor their mental health, and address any concerns or barriers to well-being.
• Facilitate effective communication among family members to ensure everyone is informed and involved in the care process.
• Maintain diligent and accurate documentation of client interactions, care plans, progress notes, and assessments in compliance with ethical standards.
• Stay updated on relevant social work practices, policies, and regulations to ensure best practices and ethical compliance.

The ideal candidate will understand the intricacies of geriatric social work, demonstrating empathy and skill in navigating the emotional complexities of aging clients and their families.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of professional experience in a social work capacity, ideally within a community center or geriatric care setting.
• Strong familiarity with social work principles, aging-related issues, and access to local community resources for seniors.
• Demonstrated assessment, counseling, and problem-solving abilities.
• Capacity to empathize and build rapport with diverse client populations, particularly seniors.
• Excellent interpersonal and communication skills for effective teamwork and client interaction.
• Proficiency in using electronic health record (EHR) systems and modern communication tools.

• Experience working in a multi-disciplinary team environment.
• Prior involvement in advocacy or community outreach initiatives tailored to senior populations.
• Familiarity with telehealth practices and virtual counseling methodologies.
