Geriatric Care Social Worker (MSW/BSW)   Location: Fairfax, CA 
  Healthcare / Long-Term Care Setting
  Full-Time | Temp-to-Hire

 Job Overview:
Synergy Health Staffing is seeking a compassionate and experienced Geriatric Care Social Worker to support older adult patients in a healthcare or long-term care setting. This role focuses on psychosocial support, discharge planning, care coordination, and helping patients and families navigate the challenges of aging.
This is a contract-to-permanent placement opportunity with strong potential for long-term employment.
 Responsibilities:
  •  Conduct psychosocial assessments for geriatric patients 
  •  Support discharge planning and care transitions 
  •  Assist families with long-term care placement decisions 
  •  Provide emotional support and crisis intervention 
  •  Coordinate services with hospitals, SNFs, hospice, and community agencies 
  •  Support patients with dementia, chronic illness, and end-of-life care needs 
  •  Maintain accurate documentation in compliance with facility and state standards 
  •  Collaborate with interdisciplinary healthcare teams 
 California State Requirements:
Candidates must meet requirements regulated by the California Board of Behavioral Sciences:
  •  Bachelor’s degree (BSW or related field) required; MSW preferred 
  •  Experience in geriatric care, healthcare, or long-term care settings 
  •  ASW or LCSW preferred for clinical roles 
  •  Knowledge of California healthcare and aging service systems 
  •  Understanding of mandated reporting requirements for elder abuse 
Preferred Experience
  •  2-3+ years in hospital, SNF, hospice, or geriatric care settings 
  •  Experience with dementia/Alzheimer’s populations 
  •  Strong discharge planning and case coordination skills 
  •  Strong communication and family support skills 
 Compliance Requirements
  •  Live Scan background check (DOJ/FBI clearance) 
  •  TB test and health clearance (as required by facility) 
  •  Mandated Reporter Training (California elder abuse reporting) 
  •  Valid identification and work authorization
