This position is based in our state-of-the-art Rehabilitation Center located in Portsmouth, Virginia, where you will work directly with patients in an onsite setting. As a Physical Therapist at Forest Manor, your essential duties will include:

• Conducting thorough evaluations of patients by reviewing medical histories, performing relevant assessments, and developing personalized treatment plans based on findings.
• Communicating evaluation results and treatment goals effectively to patients and their families, ensuring they are active participants in their rehabilitation journey.
• Implementing treatment interventions that align with the best practices in physical therapy using evidence-based approaches.
• Monitoring and documenting progress regularly, adjusting the treatment plan as necessary to accommodate changes in patient conditions.
• Collaborating with multidisciplinary healthcare teams to ensure coordinated care and timely referrals for additional services when needed.
• Participating in discharge planning and follow-up care coordination to maximize patient outcomes post-therapy.
• Maintaining accurate and timely documentation in compliance with healthcare regulations, ensuring transparency and clarity in patient care records.

• Certification in Basic Life Support (BLS) from the American Heart Association.

While prior work experience in a rehabilitation setting is not mandatory, candidates with a history of internships or clinical rotations in outpatient or inpatient physical therapy environments will be preferred.
