Job description

InGenesis is currently seeking a Pharmacist to work with our client. In this role, you will be responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of the Option Care, pharmacy practice professional standards, and applicable regulatory entities. This is an entry level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Job Duties * Determine suitability of individual patients for home care. * Design and monitor patient-specific pharmacy care plans for the pharmacotherapy of home care patients. * Modify care plans, as needed. * Provide education on medication-use. * Ensure continuity of pharmaceutical care to and from home and other patient-care settings. * Manage use, maintenance, and troubleshooting of medication administration and other equipment. * Provide concise and timely response to requests for drug information. * Manage use of investigational drug products according to established regulations and client policies. * Perform other duties as necessary. Minimum Qualifications * Graduate of an accredited School of Pharmacy. * Active Pharmacy license as issued by the State Board of Pharmacy. * Experience providing customer service, including meeting quality standards for services and evaluation of customer satisfaction. * One of the following: One of the following: (a) At least one 1 year of experience in sterile IV admixture procedures (b) knowledge of non-sterile compounding, immunizations and medication therapy management (MTM) (c) completion of an ASHP accredited Pharmacy Residency Program, (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy or (e) equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services Equal Employment Opportunity Statement: InGenesis is an equal opportunity employer committed to fostering a diverse, inclusive, and equitable workplace.
