Job description

Job Title: PRN Speech Therapist  (ST) – Home Health


Position Summary:

We are seeking a skilled and compassionate Speech Therapist (ST) to provide per diem home health services to patients with a variety of communication, cognitive, and swallowing needs. As a PRN ST, you will assess, develop, and implement individualized treatment plans to support patients in achieving their highest level of functional independence in the home environment.

This role offers flexibility, autonomy, and the opportunity to make a meaningful difference in patients' lives through one-on-one care.


Responsibilities:
  • Perform initial and ongoing evaluations of patients' speech, language, communication, cognition, and swallowing disorders.

  • Develop and implement individualized care plans in coordination with the referring physician and care team.

  • Provide therapeutic interventions and education to patients and caregivers.

  • Monitor and document patient progress, revising plans of care as needed.

  • Accurately document all visits and patient interactions in the WellSky EMR system.

  • Collaborate with other members of the interdisciplinary care team to ensure comprehensive care.

  • Ensure compliance with all state, federal, and agency clinical and documentation standards.

  • Current and active Florida ST license.

  • Minimum of 1 year of clinical experience; home health experience preferred.

  • Experience with WellSky EMR or similar home health documentation system preferred.

  • Excellent communication, organizational, and time management skills.

  • Reliable transportation, valid driver’s license, and proof of insurance (for field visits).

  • Ability to work independently in a field-based setting.
