Work Environment

This position is fully remote; however, you will coordinate closely with our hospice center located in Chattanooga, Tennessee. The standard work schedule is from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evenings or weekends required depending on client needs. Flexibility in scheduling allows us to ensure continuous care and respond to urgent situations effectively.

Key Responsibilities
  • Conduct comprehensive assessments of seniorsโ€™ psychosocial needs, focusing on emotional, mental, and social factors that affect their well-being.
  • Collaborate with clients and their families to create tailored care plans that address specific goals and aspirations, ensuring a personalized approach to support.
  • Provide ongoing counseling and emotional support, guiding clients and their families through significant life transitions and challenges.
  • Advocate for clients' rights, ensuring access to community resources and services essential for their support.
  • Work collaboratively with interdisciplinary teams, including nurses and other healthcare professionals, to coordinate and enhance the care provided to clients.
  • Conduct regular check-ins and home visits to monitor clients' conditions, assess safety issues, and respond to any concerns raised by clients or families.
