• Conduct comprehensive clinical assessments of patients, considering their psychological, social, and emotional needs.
• Develop and implement individualized treatment plans that employ evidence-based therapeutic techniques, such as solution-focused therapy, to help patients and families navigate end-of-life challenges.
• Maintain accurate and timely electronic clinical records as required by policy while ensuring confidentiality and compliance with state and federal regulations.
• Collaborate with a multidisciplinary team, including nurses, physicians, and chaplains, to create a cohesive care plan addressing all aspects of the patient’s experience.
• Provide education and support to patients and their families about mental health resources, coping strategies, and available community services.
• Stay informed about the latest developments in mental health treatments and hospice care best practices through continuous professional education and training.

In this role, you will work with a diverse patient population that includes adults facing terminal illnesses and their families, each experiencing a unique set of emotional and psychological challenges. Your expertise in mental health will be instrumental in providing the emotional support and resilience-building strategies necessary for patients to cope with their circumstances and enhance their overall quality of life during this critical time.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Virginia.
• Demonstrated knowledge of solution-focused therapy and other relevant therapeutic methodologies.
• Excellent interpersonal and communication skills, particularly in building rapport and trust with patients and families.
• Strong problem-solving abilities and a compassionate approach to care.

• Experience working in hospice or palliative care settings preferred.
