• Conduct comprehensive assessments of seniors' psychosocial needs, evaluating their emotional, social, and mental well-being.
• Collaborate with clients and their families to create personalized care plans tailored to individual preferences and goals.
• Provide counseling and emotional support to help clients navigate life transitions, such as health changes or loss of independence.
• Advocate for clients' rights and assist in accessing necessary community resources, ensuring they receive appropriate services.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Clinical Social Worker (LCSW-C) in Maryland.
• A minimum of two years of experience working as a Social Worker, particularly in a healthcare setting serving seniors.
• Solid understanding of social work principles, aging-related issues, and available resources in the community.
