During your tenure, you will engage with diverse healthcare professionals, including physicians, nurses, and care coordinators, to deliver high-quality hospice services. Though primarily remote, occasional visits to the hospice center may be required to enhance communication and collaborative care.

• Conduct in-depth psychosocial assessments, collaborating with medical teams to develop individualized care plans addressing emotional, social, and financial challenges faced by patients and families.
• Provide both individual and group counseling sessions, focusing on adjustment issues, grief, and coping strategies for patients navigating complex emotions during end-of-life care.
• Coordinate with community organizations to facilitate access to necessary post-discharge services, ensuring a seamless continuum of care.
• Develop and execute thorough discharge plans, linking patients to vital community resources that support their emotional and practical needs.
• Engage in crisis intervention and provide immediate emotional support to patients and their families during challenging moments.
