Dentist
Location: Lyons, NY
Contract Duration: 1 Year (with four optional one-year renewals)
Work Arrangement:
  • On-site dental services at County Nursing Home (192-bed facility)
  • Minimum 52 scheduled on-site sessions per year (at least 2 hours each)
  • Additional hours for emergency care or off-site hospital visits as needed
Client seeks a licensed Dentist to provide comprehensive, patient-centered dental care for long-term care residents. This part-time, contract-based role includes on-site treatment, emergency response, staff training, compliance reporting, and policy guidance in line with New York State Health Code Title X Chapter V, NYS DOH regulations, and industry standards.
Deliver preventive, restorative, prosthodontic, and emergency dental services while supporting the overall oral health and hygiene program of the facility.
Key Responsibilities
Clinical Care & Oversight
  • Conduct dental exams (initial, routine, emergency), diagnostic X-rays, cleanings, scaling, and hygiene instruction
  • Provide restorative services, extractions, fabrication and repair of dentures (1 new set per resident annually)
  • Perform limited fixed prosthetics and make referrals to dental specialists when needed
  • Offer off-site dental care requiring general anesthesia in coordination with hospitals
Program Development & Compliance
  • Serve as dental consultant and advisor to the facility administration
  • Develop and maintain dental care policies and hygiene protocols
  • Ensure dental records are updated within 7 days of each resident's comprehensive assessment
  • Maintain all documentation per NYS DOH 483.55 and CMS standards
Reporting & Training
  • Provide monthly, quarterly, and annual reports on services provided, oral hygiene status, and visit summaries
  • Deliver staff in-service training on oral hygiene practices and protocols
  • Ensure backup emergency dental arrangements and compliance with sanitary standards
Mandatory Qualifications
  • Licensed Dentist in New York State
  • Experience in nursing home or long-term care dental services (preferred)
  • Ability to provide and maintain:
    • Dental equipment and supplies (contractor-owned)
    • Licensed professional staff
    • Lab fees for all procedures
  • Adherence to infection control standards, PPE policies, and professional conduct
  • Successfully pass all background checks as required by the County
