This role involves working onsite at our state-of-the-art Rehabilitation Center, designed to promote comfort and healing for our patients. The center is equipped with the latest rehabilitation technologies and therapeutic equipment, enabling us to deliver effective treatment strategies. As part of our collaborative team, you will work closely with other healthcare professionals to ensure holistic care for our patients.

• Conduct thorough evaluations of patients' physical conditions in a home care or facility-based environment, taking into account their individual history and rehabilitation goals.
• Develop and implement personalized treatment plans utilizing a range of therapeutic techniques, including exercise regimens, manual therapy, and modalities to optimize recovery and enhance mobility.
• Monitor patient progress regularly by keeping detailed records of treatments, outcomes, and any adjustments to therapy plans based on patient response.
• Provide education and support to patients and their families on rehabilitation practices, including the use of assistive devices and self-care techniques to foster independence.
• Collaborate with multidisciplinary teams to consult on patient cases and share knowledge with caregivers and health personnel to ensure comprehensive care continuity.
• Engage in continuous professional development activities, such as attending workshops and staying updated with the latest research in physical therapy.

Your work will involve treating a diverse patient population, including children, adults, and seniors, each presenting unique challenges in their rehabilitation journey. You will adapt your therapeutic approaches to meet the variances in diagnoses and recovery stages, ensuring that every patient receives personalized care that aligns with their specific needs.

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Current, valid license to practice as a Physical Therapist in the state of Florida.
• Exceptional communication skills, both verbal and written, with the ability to connect with patients and collaborate with a multidisciplinary team.
• Proficiency in using electronic health records and standard office software; familiarity with rehabilitation technology is a plus.
• A demonstrated ability to display flexibility and adaptability in a busy clinical environment.

• At least 2 years of experience in a physical therapy role, with exposure to home care settings or rehabilitation facilities.
• Previous experience working with a variety of patient diagnoses, particularly orthopedic and neurological conditions.
• Familiarity with evidence-based practice methods and emerging trends in physical therapy.
