Interact Care Wellness is seeking a Licensed Professional Counselor (LPC) to join our dynamic team in a full-time, remote capacity. Key Responsibilities
  • Conduct comprehensive assessments and ongoing evaluations to tailor care plans that meet the unique needs of each patient.
  • Facilitate communication among supervisors, direct care staff, family members, and other stakeholders regarding patient progress and adjustment of care as necessary.
  • Manage a tentative schedule that includes appointments, home visits, and monitoring observations to track patient progress and adherence to care plans.
  • Coordinate home visits that comply with regulatory standards for in-home assessments, and support documentation, ensuring a thorough analysis of patient conditions.
  • Implement and monitor the effectiveness of treatment plans, making adjustments based on patient feedback and clinical outcomes.
  • Participate actively in Interdisciplinary Team Meetings to foster collaboration and share insights about patient care across disciplines.
  • Prepare detailed monthly and quarterly progress notes to keep an accurate record of patient development and care strategies.
  • Facilitate training sessions to ensure all team members are proficient in implementing care plans and maintaining appropriate documentation.
  • Conduct follow-up interviews, home visits, and observations to provide continued support and encouragement for clients' ongoing needs.
  • Serve as a liaison between the clinic and the mental health community, promoting initiatives that align with development goals and enhance service offerings.
  • Assist in creating and refining training materials aimed at improving program delivery and patient care.

Compensation and Benefits
