Primary Care Physician (Full-Time)
Schedule: Monday-Friday, 8:00 AM - 5:00 PM (No weekends or hospital rounds)
Setting: 100% Outpatient (Geriatrics-focused position)

About the Role
We are seeking a compassionate and dedicated Primary Care Physician to deliver high-quality, value-based care to an established panel of predominantly geriatric patients. This role emphasizes preventive medicine, chronic disease management, and building meaningful, long-term patient relationships in a supportive outpatient environment.
You will work within a collaborative, multidisciplinary care model designed to improve patient outcomes, enhance access to care, and reduce unnecessary hospitalizations. Our approach prioritizes personalized care plans, proactive health management, and patient education.
Key Responsibilities
  • Provide comprehensive primary care services, including evaluation, diagnosis, and treatment of acute and chronic conditions.
  • Deliver preventive care, wellness visits, and health screenings with a focus on early intervention.
  • Develop and manage individualized care plans for a predominantly geriatric population.
  • Collaborate closely with nurse practitioners, care coordinators, and support staff to ensure seamless, patient-centered care.
  • Utilize electronic health records (EHR) to document clinical encounters and track patient outcomes.
  • Engage in quality improvement initiatives and contribute to a culture focused on continuous care enhancement.
  • Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices.
Qualifications
  • MD or DO from an accredited medical school
  • Internal Medicine or Family Medicine or Geriatric Medicine
  • Active, AL license
  • Current DEA registration
  • At least 3 years of experience in Outpatient Primary Care
  • Strong clinical judgment with a focus on preventive and longitudinal care
  • Excellent interpersonal and communication skills
  • Commitment to a team-based, patient-first care model
