Overview
Per Diem Rates:
The telehealth visits for community = $150 (which is an average compensation).
In person visits are compensated = $150 - $210 (with $180 being an average FFS)
Orientation hourly rate: $90/Hour
This field position will cover Brooklyn and Queens regions.

Responsibilities
Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.
In collaboration with physician, the NP will provide comprehensive assessment, including multifactorial complex
physical examination and psychosocial needs, self - management education and tools to better manage patient's multiple
conditions. As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary
care services to an assigned the at risk panel of Elderplan members who reside in the community. The NP will insure
development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. The NP will act as clinical resource to the member, family and interdisciplinary team.
  • Provide and/or coordinate primary clinical care services for assigned high risk
    members and facilitates supportive care during transitions from acute or sub-acute care setting
  • Acts as a team player, member's advocate, and a clinical lead while working closely with
    Interdisciplinary Team to coordinate member's care and optimize health outcomes
  • Acts as resource and contact for member/designated representative regarding all aspects of member
    individualized care plan and disease process

Qualifications
  • Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
  • One to two years clinical experience in role of nurse practitioner required
  • Minimum one year in geriatric specialty
  • Previous clinical RN experience in acute, subacute and/or long term care setting preferred
  • Prior experience in manage care a plus
  • NYS NP License, Registration, DEA certificates
  • Driver license and car required
