LIBERTY SENIOR LIVING is currently seeking an experienced:
TRANSPORTATION COORDINATOR - (CNA/EMT)
JOB SUMMARY:
  • After notification by facility staff of needed appointment, will make all appointments for residents.
  • Will maintain a calendar of all appointments so that none will conflict.
  • Will follow all safety guidelines to ensure resident transports are performed in the safest possible manner.
  • Will maintain required logs of each transport and for each vehicle.
  • Completes Quality Assurance monitors according to policies and procedures. .
  • Will establish and maintain a maintenance schedule for each vehicle that is used in transports.
  • Assists in maintaining a good working relationship with the community, hospital, physician offices and other health care facilities and agencies and assuring coordination of services for our residents.
  • Perform Receptionist position when necessary
  • Perform resident care when assigned and appropriate based on certification.
  • Perform other duties as assigned.

JOB REQUIREMENTS:
  • Must be at least 21 years of age or older, must be of good moral character, and have a genuine concern for the elderly and disabled.
  • Must have CPR, First Aid Training, and wheelchair safety training. Preferably certified nursing assistant.
  • Must have skill checklist completed by Liberty trainer prior to transporting residents.
  • Must have a valid state Driver's License to operate a motor vehicle with an appropriate driving record.
  • Must be willing to work with various department directors, patients, visitors, family members, and community health care providers.
  • Must have the ability to plan, organize, and follow-up on work assignments.
  • Must have the ability to make independent decisions and work well under pressure.
  • Must have the ability to be flexible in performing different tasks.
