Job description

The Sr. DSP Medical Coordinator is responsible for all of the duties of a Direct Support Professional plus scheduling, tracking and documenting all medical appointments, as well as scheduling all labs and required tests.

  • Document all paperwork required, including incident reports, log entries and Res. Hab. progress notes when appropriate
  • Track all medical appointments and follow-up appointments and document in tracking forms
  • Track all lab tests, as well as other tests and ensure correct documentation in medical tracking log
  • Document all consults, refusals and MD recommendations appropriately
  • Ensure that medication inventories are completed at least weekly, ad will order new medication as prescribed, in a timely manner, and maintain the medication storage area in accordance with regulations
  • Identify medication errors and communicate the errors to the nurse and manager effectively
  • Communicate with the nurse and manager via a Medical Communication Log
  • Follow all AMAP procedures
  • Attend ISP or other meetings when requested
  • Maintain open lines of communication with management and staff

  • NYC Driver license preferred, but not required.
  • HS Diploma or GED required
  • 1+ year of HHA experience required, CNA experience a plus
  • 2+ years of Direct Support Professional experience a plus
