About the Role:
As a Case Manager in home health care, you will coordinate services with other healthcare professionals, communicate with insurance providers, and oversee visiting nurses. Your responsibilities extend to crafting individualized Plans of Care (POC) based on patient needs and family input, supporting treatment plan adherence, and monitoring overall well-being, medical requirements, and specialist treatments. Additionally, you'll recommend necessary community services. Your duties also encompass collaborating with the primary doctor to implement and adjust the plan, ensuring thorough documentation of all required information.

Job Responsibilities:

  • Coordinating home healthcare services in office.
  • Collaborating with healthcare providers, physicians, and different insurances.
  • Developing and implementing individualized Plans of Care (POC) based on patient needs and family feedback.
  • Supporting adherence to treatment plans and monitoring overall patient wellness.
