Diversity Inclusion Program Coordinator Jobs (NOW HIRING)

The Program Coordinator works directly with adults with intellectual and developmental disabilities to coordinate their services, employment goals, and required documentation. This position manages an assigned group of clients and works closely with clients, families, guardians, case managers, residential providers, employers, and Merrick's program teams. The Program Coordinator regularly visits clients at their community job sites to check in, assess how employment is going, help determine whether a position is a good fit, and identify additional support that may be needed. This position also participates in meetings with clients and their support teams and provides backup support to other positions within the Employment Services department.

Responsibilities

1. Develop, write, monitor, and evaluate individualized client goals and outcomes.
2. Complete and maintain client files, assessments, documentation, reports, and other required paperwork.
3. Meet regularly with clients and their support teams, including families, guardians, case managers, residential providers, and other service providers.
4. Conduct visits to clients at their community job sites to check in on their employment, identify concerns, and help evaluate whether the job continues to be a good fit.
5. Maintain professional communication with clients, their support teams, employers, and internal program staff.
6. Submit waiver, funding, and rate documentation and follow up to ensure accurate Service Agreements are in place.
7. Coordinate client transportation needs and other services as needed.

Qualifications

• Must be a Qualified Developmental Disability Professional (QDDP) or Designated Coordinator as defined in federal and/or state statute as follows:
  1. A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older; or
  2. An associate degree in a field related to human services and two full years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older, or
  3. A diploma in a field related to human services from an accredited post-secondary institution, and three full years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
  4. Four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications listed above. 
• Excellent written, verbal, and customer service skills.
• Strong organization, time management, and problem-solving skills.
• Ability to independently manage multiple clients, deadlines, meetings, and documentation requirements.
• Ability to use Microsoft applications, databases, and internet-based systems.
• Schedule flexibility to attend client meetings and job-site visits throughout the Minneapolis–Saint Paul metropolitan area.
• Valid Class D driver's license and required personal vehicle insurance.

Essential Physical Functions

The employee must be able to continually sit, stand, walk, and communicate throughout the day and be able to stoop, bend, kneel, twist, run, reach, push, and pull. The position requires regularly moving up to 10 pounds and occasionally moving up to 50 pounds, including assisting with clients and wheelchairs as needed.
