Work Environment

This role is entirely remote, allowing for flexibility in your work schedule while ensuring that clients receive effective support from the comfort of their own spaces. You will collaborate with a multidisciplinary teamโ€”including physicians, occupational therapists, and psychologistsโ€”via telehealth platforms, ensuring that service delivery is seamless and conducive to patient care.

Key Responsibilities
  • Conduct initial client assessments to understand their unique goals, preferences, and challenges, followed by crafting tailored rehabilitation plans to guide their journey.
  • Design and implement individualized treatment strategies that consider each client's educational background, strengths, limitations, and future aspirations, adjusting plans as necessary to maximize effectiveness.
  • Engage proactively with team members from various disciplines to design and execute integrated rehabilitation programs aimed at meeting the specific needs of each client.
  • Maintain comprehensive and accurate client records, documenting demographic information, eligibility criteria, and treatment plans, as well as keeping notes on ongoing communications and interventions.
  • Regularly monitor client progress, documenting outcomes to assess the effectiveness of treatment and identifying necessary adjustments to achieve target goals.
  • Provide ongoing support and communication with clients throughout their engagement in therapy, job training, and occupational placement, and swiftly address any challenges that arise.
  • Work collaboratively with family members and support networks to build effective strategies that enhance the overall client experience and address any additional needs.
  • Conduct needs assessments and prepare timely reports to communicate client status and progress to stakeholders.
  • Offer guidance and direct clients to relevant resources and support systems within the community as appropriate.
Patient Population / Client Focus

Your primary focus will include working with individuals grappling with mental health issues, mood disorders, and emotional challenges, both in outpatient scenarios and during transitional care periods. Your empathetic approach will be essential in helping clients regain control over their lives, build coping mechanisms, and thrive socially and professionally.

Qualifications
  • A master's degree in counseling, social work, psychology, or a related field is required.
  • Current licensure as a Licensed Professional Counselor (LPC) is essential.
  • Experience in providing mental health treatment to clients with various mental health conditions in both inpatient and outpatient contexts is preferred.
  • A collaborative mindset and a commitment to fostering positive change within the client population.
  • Strong clinical skills, including proficiency in evidenced-based methodologies such as motivational interviewing, cognitive-behavioral therapy, and dialectical behavioral therapy.
