Director of Nursing - Assisted Living Facility

Job Summary: 

The Director of Nursing (RN) is responsible for ensuring the delivery of high-quality resident care and clinical services in compliance with Wisconsin Assisted Living Facility regulations. This position provides leadership to the care giving team, oversees clinical programs, maintains regulatory standards, and collaborates with the Executive Director to achieve organizational goals related to resident wellness and satisfaction.

Key Responsibilities:

  • Resident Care & Clinical Oversight
    • Develop and monitor individualized care plans in line with resident needs and regulations
    • Conduct assessments, maintain medical records, and oversee medication management
    • Lead infection control efforts and manage on-call clinical support for emergencies or changes in condition
  • Regulatory Compliance & Risk Management
    • Ensure adherence to Minnesota Assisted Living Facility regulations (Minn. Stat. Ch. 144G)
    • Conduct internal audits and support state surveys
    • Investigate and report incidents or suspected abuse per state guidelines
  • Staff Leadership & Training
    • Supervise, train, and evaluate care staff and medication aides
    • Provide initial and ongoing education on health, medications, and care procedures
    • Ensure staff competency in delegated nursing tasks and adherence to care protocols
  • Communication & Collaboration
    • Partner with the Executive Director and interdisciplinary team to deliver high-quality resident services
    • Maintain clear communication with residents, families, and healthcare providers
    • Participate in leadership meetings and contribute clinical insight to operational planning
  • Administrative & Quality Oversight
    • Maintain accurate clinical documentation and resident files
    • Monitor clinical indicators and participate in quality improvement efforts
    • Assist with staff scheduling and coordinate care services

Qualifications and Requirements:

  • Current, unencumbered Registered Nurse (RN) license in the State of Wisconsin.
  • Minimum of 2–3 years of clinical experience in senior living, long-term care, or related healthcare setting.
  • Previous leadership or supervisory experience required; experience in an Assisted Living facility strongly preferred.
  • Thorough understanding of Wisconsin ALF regulations, including medication management and resident rights.
  • Strong leadership, problem-solving, and organizational skills.
  • Proficient in care planning, clinical assessment, and documentation procedures.
  • Excellent communication skills and ability to work collaboratively with diverse groups.
  • Ability to work flexible hours as needed, including weekends or evenings for on-call coverage.
  • Must pass background checks and any other pre-employment screening as required by state regulations and company policy.
