About the Role

As a Director of Community Partnerships, you will play a key role in expanding our reach by developing strong partnerships with healthcare professionals, senior living communities, and local organizations. You will be responsible for building relationships, creating opportunities, and driving meaningful growth in your community. Responsibilities

  • Develop and execute a strategic community partnership plan to generate new client referrals and increase census growth

  • Identify and cultivate new referral relationships that turn into consistent business

  • Meet and exceed monthly and quarterly growth targets

Build Powerful Community Relationships

  • Establish and grow partnerships with:

    • Home Care, Home Health, and Hospice providers

    • Hospitals, physician offices, and discharge planners

    • Assisted Living and Senior Living communities

    • VA programs, senior centers, and local community organizations

Be a Trusted Resource

  • Maintain strong relationships with referral partners, clients, and families

  • Serve as a trusted liaison and resource within the healthcare and senior care community

  • Ensure a smooth onboarding and handoff experience for new clients

Represent the Brand

  • Represent CareBuilders at Home at networking events, community meetings, and health fairs

  • Increase brand awareness and position our agency as a trusted and preferred provider

Track Success

  • Monitor marketing activity, referrals, and conversions

  • Provide regular updates on pipeline growth and partnership activity

  • Collaborate closely with operations and care teams to ensure excellent service delivery

Preferred Experience

  • Experience using CRM or referral tracking systems preferred

Requirements
