Job description

Job Objective(s):

Responsible for all aspects of services to the residents including coordination of internal services (including health services, dining services and facilities services), oversight to activities and events, and communication to residents and their families.

Qualifications

  • Four-year degree in a related field plus at least two years experience working in Senior Housing and/or Assisted Living or at least 5 years direct experience in Senior Housing or Assisted Living Services
  • Licensed Assisted Living Director (LALD) certification or willingness to obtain within 2 years
  • Experience in effectively relating to an aging population
  • Knowledge of the Housing with Services model of service delivery as it is structured in the State of Minnesota under MN Statute 1444G
  • A healthcare background and/or familiarity with medical conditions and disabilities
  • Significant knowledge of community resources and care options
  • Knowledge of supervisory practices and principles
  • Effective communication skills, both oral and written. Must have a comfort level with public speaking and community network as well as the ability to form trusting relationships one on one with clients
  • Skill in the use of computer programs
  • The ability to facilitate resident and family conferences
  • Able to read, write, spell, do basic math, speak and understand English

Job Responsibilities and Duties

  • Adhere to all company personnel directives as per the Employee Handbook and ensure that all employees do the same.
  • Maintain strong positive relationships with potential residents, families and caregivers of potential residents, community contacts, current residents and staff, the Board of Directors, and Great Lakes Management. Represent the organization in a manner that is consistent with its mission and values.
  • Supervise staff, giving direction, administering any needed correction, and evaluating performance on a regular basis. Evaluate programming (Life Enrichment) to meet current and future needs.
  • Assist in identifying additional staffing needs and assist with the recruitment and evaluation of potential departmental employees as needed.
  • Work directly with clients as needed in conjunction with staff to address the individual needs of clients and to assist with their housing decision. Coordinate the continuum of personal and health services to assure appropriate placement within our facility. Contact families in event of resident health or service concerns, keeping Executive Director and Director of Nursing informed of the situation.
  • In coordination with nurses, coordinate with hospital and nursing home social workers for the discharge and placement of residents. Meet routinely with the Director of Nursing to discuss resident concerns.
  • Participate in the pre-admission process to ensure that potential residents are appropriate according to stated residency requirements.
  • Market programs to present and prospective residents who will require services to live independently and to make appropriate referral for services.
  • Assist with information about community resources to residents and family. (Podiatry, transportation, external services, etc.)
  • Welcome and orient new residents upon move-in with service information. Oversee the orientation process (one-one meetings, group meetings, welcome person) and direct staff in assisting the transition for new residents.
  • Complete and process all paperwork associated with Resident Services, with record keeping and written communication to staff that is defined by current Great Lakes Management procedures. Complete this process in a timely manner. Provide weekly report to Administrator regarding supportive service activity and changes.
  • Develop procedures for tracking of service clients to ensure that all residents receive adequate follow-up and attention. Work with staff members to provide consistency and organization to information tracking.
  • Assist in the development of the Life Enrichment budgets.
  • Develop and coordinate, in consultation with the Campus Administrator and the Resident Services team, all internal communication. This includes any modifications or updates to current Service materials as well as the development of new materials.

Must be able to frequently perform tasks such as, but not limited to, those listed below:

  • Must be able to move throughout the workday and throughout the building.
  • Must be able to cope with the mental and emotional stress of the position.
  • Move, push, pull and/or lift objects weighing 10 to 25 pounds.
  • Use hands and arms for simple grasping, pushing, and pulling
  • Perform tasks requiring actions of muscles or groups of muscles to walk, stoop, bend, lift, and to carry objects
  • Walk and/or stand for prolonged periods of time
  • Speak clearly and make self-understood in face-to-face interactions
  • See objects closely to read directions, instructions, etc.
  • Hear normal sounds and voices with some background noise to receive verbal instructions, answer telephones, communicate with others, etc.

Environment

Works indoors in a climate-controlled environment majority of the time.

Working Hours

Typically, Monday through Friday 8:30am – 5:00pm. The work schedule and hours may vary from week to week. You may be required to work beyond the normal schedule which includes weekends and holidays.
