Duties
  • Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
  • Monitor residents’ health by taking vital signs, observing for changes in condition, and reporting findings accurately while maintaining HIPAA confidentiality.
  • Prepare nutritious meals aligned with individual care plans and dietary restrictions; ensure cleanliness of the dining area and kitchen.
  • Administer medications following prescribed protocols and document medication administration meticulously to ensure compliance with healthcare regulations.
  • Support residents with memory care needs such as Alzheimer’s or dementia by implementing specialized care strategies and engaging them in meaningful activities.
  • Maintain a safe environment through regular cleaning, sanitation, and adherence to safety procedures; assist with light housekeeping tasks as needed.
  • Collaborate with healthcare professionals, social workers, and family members to develop and update personalized care plans that meet each resident’s unique needs.
Requirements
  • Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
  • Knowledge of patient observation techniques, vital signs measurement, medication administration, and emergency response procedures such as CPR and first aid.
  • Experience caring for individuals with developmental disabilities, autism spectrum disorder (ASD), Alzheimer’s disease, dementia, or hospice needs is advantageous.
  • Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or resident lifts when necessary.
  • Strong communication skills to effectively work with residents, families, healthcare teams, and community resources.
  • Demonstrated understanding of HIPAA regulations to protect resident privacy and confidentiality.
  • Flexibility to work various shifts including evenings, weekends or overnight hours as required for 24/7 care environments.
